Can I designate one or more people to receive my life insurance?

Yes, you can designate your life insurance as you choose. You may assign it to your Estate, to individual(s), or designate an institution to receive the benefit in the event of your death. For more information on how to do this, please review page 2 of the Appointment/Change of Beneficiary for Group Life (Dependent Life and AD&D) form provided by your employer.

Remember to return the completed form to your employer to ensure their records reflect your wishes. It’s also a good idea to periodically review your designations to confirm they are up to date and remain in keeping with your wishes. The employer must retain the original signed Appointment of Beneficiary form(s) in accordance with their document retention policy until a claim has been made.
