How do I apply for LTD benefits?
If you continue to be unable to work due to an accident or sickness and you are approaching the end of the 5-month LTD qualification period, contact your employer immediately. If you are eligible, they will provide you with the appropriate LTD claim package with forms for yourself and your physician to complete.
Gather all the required information and submit it to the appropriate Canada Life office listed on the claim forms. Keep a copy of the information submitted. Canada Life will contact you directly with the decision, or whether they require more information, or have any questions.
