Group Life and Accidental Death or Dismemberment
- Who is my Group Life and Accidental Death & Dismemberment (AD&D) coverage with?

How will my beneficiary receive my Life/AD&D benefit?
In the event of your death, the benefit will not be paid automatically. Your beneficiary(ies) or Executor(s) should contact your employer for assistance in filing a claim.
Your employer will provide your beneficiary(ies) with the appropriate Proof of Death claim form. The “Statement of Claimant” section should be completed by the appropriate party(ies) making a claim for the benefit.
In the event death was accidental, your beneficiary(s) or Executor will also need to provide a police or workplace accident report, and a Coroner’s Report or Autopsy Report in order to make a claim for both Life and Accidental Death benefits.
Once the claim(s) has been accepted, Canada Life will send the payment via the employer, who will arrange for the beneficiary(ies) or executor(s) to receive the cheque.
